Powers Lake Public School is partnering with Northland Health Centers to offer students sports physicals on Monday, July 27 from 9:00 a.m. – 5:00 p.m. To schedule a sports physical call the Ray Clinic at 701.568.5600
Students must have forms completed before their appointment by a parent and/or guardian. Forms can be found online at northlandhealthcenters.org/sports-physicals/.
What payments are accepted?
· ND BCBS Insurance
· ND Medicaid
· Cash payment of $30 due at time of appointment.
